Booking Form

Now that you know a little bit about me, here is your chance to introduce yourself. Nothing works without effective communication, so I've made it super easy for new friends to contact me.

*Screening is REQUIRED!* Yes! While I appreciate your need for discretion, my safety is just as important as your privacy. Trust is an integral part of our relationship.
 If I cannot verify you, I wont see you.

Please fill out the booking request form below. This is a safety precaution & serves as a primer so I can best tailor your session around your specific needs & interests. Incomplete requests will take longer to process or may not receive a response. If you don’t want to fill out any forms or provide appropriate information, you will not be able to schedule a session with me. Gentlemen who fill out my form, get the highest priority from me.

In addition to your personal information, If you have any references or a member of any provider boards, please provide:
- At least 2 recently active references that you have seen with a year
- If you don't have any references, you can provide proof of employment and/or career credentials.

*Please let your references know that you are using them as a reference before sending their info.

Upon receiving your form, you will be ask for a photo of yourself & a Valid ID, Driver's License, Passport, or Military ID. Any forms or information are strictly confidential & will not be stored or shared with any other sites or providers.

Make sure to provide ALL the following info the best you can. The more information that you provide, the better I can gauge our compatibility. This will help make the booking process run smoothly and easily for both of us.
